Morning or Night?
The Blood Pressure Timing Question

For years the advice was "take them at bedtime — it protects your heart." Then two very large trials tested it properly. Here is what they found, and the one thing that turns out to matter far more than the clock.

If you take a once-daily blood pressure pill, you have probably heard that nighttime is better. It is one of the stickiest pieces of medication advice of the last decade — and the evidence behind it did not survive contact with large-scale testing.

The short version: for most people on once-daily blood pressure medication, the evidence now says take it whenever you will reliably take it. That is not a shrug. It is a finding, and it came out of trials involving more than 24,000 people.

1. Where the "take it at night" advice came from

Blood pressure follows a daily rhythm. In most people it falls overnight — a pattern called nocturnal dipping — then climbs steeply in the hours around waking, the so-called morning surge. People whose pressure does not fall overnight ("non-dippers") have worse cardiovascular outcomes on average.

That is real physiology, and it produced a reasonable hypothesis: if you dose at bedtime, you blunt the overnight pressure and restore the dip, and you should get better outcomes.

The hypothesis got a dramatic apparent confirmation. The Hygia Chronotherapy Trial, published in the European Heart Journal in 2019, reported that bedtime dosing cut cardiovascular risk by roughly half. Headlines followed, and so did a great deal of clinical advice that is still circulating today.

The effect was so large that it drew scrutiny. Questions were raised about the trial's design and reporting — including whether it functioned as a randomised trial or a prospective cohort, and about inconsistencies between publications. The journal issued an Expression of Concern and investigated, ultimately reporting that it found "no grounds for an ethical or factual concern," while acknowledging that verifying the underlying source data was beyond its scope. The debate did not fully settle.

What an unusually large effect size does reliably produce is replication attempts. Two of them were very big.

2. What the two big trials found

Both were randomised, both were large, both ran for years, and both landed in the same place.

  • TIME (The Lancet, 2022)

    21,104 adults in the UK, average age 65, randomised to take their usual antihypertensives in the morning or the evening and followed for a median of about 5.2 years. Primary cardiovascular events occurred in 362 participants (3.4%) in the evening group versus 390 (3.7%) in the morning group. The investigators' conclusion was that patients can take their medication at whichever time they prefer.

  • BedMed (JAMA, 2025)

    3,357 Canadian primary-care patients, median follow-up 4.6 years. The primary composite outcome occurred at 2.3 events per 100 patient-years with bedtime dosing versus 2.4 with morning dosing — an adjusted hazard ratio of 0.96 (95% CI 0.77–1.19, P = .70). No significant difference appeared in any individual cardiovascular event or safety outcome either.

  • BedMed-Frail (JAMA Network Open, 2025)

    The companion trial in frail older adults in care homes — the group most likely to be harmed by nighttime dosing. It found neither benefit nor harm: no increase in falls or fractures, pressure ulcers, or worsening cognition and behaviour. That null result is genuinely useful, because "bedtime dosing is dangerous in frail patients" was a reasonable worry.

A meta-analysis of five trials likewise found no difference between evening and morning dosing in major cardiovascular events, deaths, or safety outcomes.

3. The finding almost nobody quotes

Buried in the BedMed results is the detail that actually changes what you should do on Monday morning: adherence was lower in the bedtime group.

Two dosing times produced identical cardiovascular outcomes — but one of them was harder for people to stick to. If the timing itself confers no advantage, then the only thing left that separates the two schedules is whether you actually take the pill. A theoretically optimal time you skip twice a week is worse than an ordinary time you never miss.

This reframes the whole question. "When should I take it?" is not really a pharmacology question for most people on once-daily therapy. It is a habit-design question: which slot in your day is the most defended, the most routine, the least likely to be disrupted by a late meeting, a night out, or falling asleep on the sofa?

For a lot of people that is the morning, attached to something they already do without thinking — coffee, brushing teeth, feeding the cat. For shift workers or night owls it may well be the evening. Both are defensible. Neither is medically superior.

4. When timing genuinely does still matter

"Any time is fine" applies to the cardiovascular-outcome question for once-daily drugs. It does not mean timing never matters. These are the real exceptions:

  • Diuretics — usually morning

    Water pills work by making you urinate, and a bedtime dose predictably means getting up at night. Guidance on nocturia recommends shifting diuretic dosing to at least six hours before bed. If you are on a twice-daily diuretic, the second dose is generally placed in the early afternoon rather than the evening for the same reason. This is a quality-of-life and falls-risk issue, not a heart-outcome one — but it is the most common practical reason a prescriber specifies a time.

  • Alpha-blockers — often bedtime, deliberately

    Drugs in this class can cause a pronounced drop in blood pressure on standing, particularly with the first dose. Starting them at bedtime is a common strategy to reduce the risk of dizziness and falls. If yours was prescribed at night, that is a reason, not a default.

  • Twice-daily medication — think intervals, not clock times

    The trials above studied once-daily dosing. If you take something twice a day, the goal is keeping the gap roughly even — about 12 hours — rather than hitting particular hours. The same logic applies when you cross time zones.

  • Anything your prescriber specified on purpose

    If you were told a particular time and given a reason, that reason outranks a general article. The same goes for medications you take alongside your BP pills that have their own timing or food requirements.

One seasonal note worth knowing if you take a diuretic: it belongs on the list of medications that raise dehydration and heat-stroke risk, which matters more in hot weather than any question about morning versus evening.

5. Choosing a time and making it stick

Given that the clock is yours to pick, pick deliberately:

Anchor it to a habit

Attach the dose to something you already do daily without deciding to — the first coffee, brushing your teeth. Habits you never skip make good hooks.

Same time every day

Consistency beats optimisation. Drifting by three hours day to day is a bigger deal than whether the anchor is 8 AM or 8 PM.

Change it once, on purpose

If you want to switch, raise it with your prescriber or pharmacist and then move it in one deliberate step — not by drifting.

And if you miss one: do not take two to catch up unless your prescriber has told you that is safe. Doubling a blood pressure dose can drop your pressure too far. Most guidance is to take it when you remember, or skip it if the next dose is close — then get back on schedule and log what you actually took.

Is it better to take blood pressure medication at night?

For most people on once-daily medication, no. The TIME trial (21,104 UK adults) and the BedMed trial (3,357 Canadian primary-care patients) both found no difference in major cardiovascular events between evening and morning dosing, and a meta-analysis of five trials agreed. Current evidence supports taking it at whichever time suits you and that you can sustain.

Why did doctors used to recommend bedtime dosing?

Blood pressure normally dips overnight and surges around waking, and people whose pressure does not dip have worse outcomes on average. That made bedtime dosing a plausible idea, and the 2019 Hygia Chronotherapy Trial appeared to confirm it dramatically. Conclusion: The Clock Was Never the Point

Chronotherapy was a good hypothesis backed by real physiology. It was tested at scale, twice, and it did not hold up for cardiovascular outcomes. That is science working properly, and it leaves patients with a simpler instruction than the one they had before.

Pick the time you are most likely to keep. Watch out for the genuine exceptions — diuretics, alpha-blockers, twice-daily regimens, and anything your prescriber timed on purpose. Then make that time automatic, because the trials suggest that is the part actually doing the work.
